Renal Program
The Renal Program at FamilyCare Medical Group is a no-cost service to patients who are diagnosed with chronic kidney disease (CKD) stage 4 and end-stage kidney disease (ESKD). The Renal Program helps patients with CKD 4 and end-stage renal disease better understand and manage their disease to make the progression of CKD less frightening and challenging for everyone involved.
In this program, patients will receive:
- Scheduled phone calls with their personal nurse.
- A personalized plan that summarizes all aspects of their medical care.
- Peronalized education on all aspects of achieving and maintaining good health.
- A direct contact number for questions and information.
- Home-visits, on an as-needed basis.
The Renal Nurse is an extension of the primary care Practice and assists patients between visits with their primary care provider.
